Boston Scientific's Watchman Slowdown Raises Utilization Questions, Oppenheimer Says

Boston Scientific (BSX) guidance cut was driven by weakness in its Watchman and electrophysiology or EP businesses, with the gap versus initial long-range targets widening into 2027, Oppenheimer said in a note Thursday.US Watchman growth is slowing sharply, and it's unclear whether this reflects fewer implanting physicians or lower utilization, the note said, with no recovery assumed for 2027 and competitive share erosion expected to rise.EP revenue beat consensus, though Oppenheimer questioned whether new products are differentiated enough to offset competition.Intravascular lithotripsy was cited as a key growth driver ahead, while the core portfolio, led by Urology and CRM, is expected to stay stable.Oppenheimer rated the stock outperform, with the price target of $85.Price: $45.26, Change: $-0.78, Percent Change: -1.71%

Boston Scientific Lowered Full-Year Guidance on Headwinds in Electrophysiology, Watchman Portfolios, RBC Says

Boston Scientific (BSX) reported Q2 revenue and EPS that beat expectations but lowered its full-year guidance, which was largely expected given known headwinds in its Electrophysiology and Watchman portfolio, as well as the base business generating 75% of the revenue growing at a rate of 6%, RBC Capital Markets said in a note Wednesday.Management attributed the slowdown in Q2 Watchman procedures to compounding clinical evidence over the past nine months affecting referral patterns, particularly for soft-indication patients, as well as operational inefficiencies from the rapid adoption of Concomitant crowding out standalone growth, according to the note.RBC said that H2 global Watchman adoption is expected to decline mid-to-high single digits, with US Q3 adoption declining mid-single digits sequentially.Boston Scientific expects H2 global Electrophysiology adoption to be flat, with US Q3 volume declining mid-single digits sequentially. RBC sees management's EP commentary as positive and somewhat offsetting Watchman given the visibility into out-years.RBC expects 2026 sales of $21.32 billion and adjusted EPS of $3.30.RBC kept an outperform rating on Boston Scientific and lowered its price target to $70 from $85.Price: $45.33, Change: $-0.71, Percent Change: -1.54%

Boston Scientific Cuts Outlook Amid Weak Demand for Watchman Heart Device

Boston Scientific (BSX) lowered its full-year guidance below Wall Street's estimates on Wednesday, citing factors including weak demand for its Watchman heart implant. The medical device maker now expects 2026 adjusted earnings per share of $3.28 to $3.32, down from its previous outlook of $3.34 to $3.41. The FactSet-polled consensus is for $3.36.Full-year net sales are now projected to grow 5.5% to 6.5% from last year, compared with its prior growth estimate of 7% to 8.5% growth. Analysts in a FactSet survey are projecting sales of $21.59 billion, indicating a 7.6% year-over-year growth."Our guidance reduction is concentrated in two areas. First, Watchman, where the US market has slowed sharply and unexpectedly, primarily driven by compounding clinical evidence, which has impacted referral patterns," Chief Executive Mike Mahoney said during an earnings call, according to a FactSet transcript. "And second, (electrophysiology), where we did not anticipate the degree of competitive share movement we're now seeing in the US market."Watchman underperformed expectations in the second quarter, with US sales growing 3% organically, according to a slide deck. Electrophysiology revenue rose 9.1% globally, but was weighed down by competitive pressures in the US.Mahoney flagged a downbeat demand outlook for Watchman for 2026, with global sales likely to be flat to up low single digits."We anticipate that year-over-year concomitant (procedure) growth will materially slow in the second half due to higher comps from 2025," Mahoney said. "Our outlook does not anticipate any improvement in standalone procedural growth trends in the second half."Boston Scientific expects flat second-half global growth for electrophysiology in the US, Mahoney told analysts.Johnson & Johnson (JNJ) raised its full-year outlook earlier this month as its innovative medicine and medical device businesses drove a strong second quarter. For the third quarter, Boston Scientific projects adjusted EPS of $0.80 to $0.82 and net sales growth of 3% to 5%. Analysts are looking for $0.84 and $5.38 billion, respectively. The company reported June-quarter adjusted EPS of $0.86, up from $0.75 a year earlier and surpassing Wall Street's $0.83 estimate. Net sales grew 7.5% to $5.44 billion, while analysts expected $5.36 billion.Sales in the cardiovascular business increased 8.3% year over year to $3.62 billion. The MedSurg division delivered a 5.9% jump to $1.82 billion, led by a double-digit gain in neuromodulation.Boston Scientific's shares rose 0.7% intraday Wednesday, reversing course from earlier in the day. The stock has plunged 51% so far this year.Price: $46.43, Change: $+0.37, Percent Change: +0.80%

Boston Scientific Has Underappreciated Growth Drivers, UBS Says

Boston Scientific (BSX) has underappreciated growth drivers beyond its electrophysiology and Watchman franchises that should support continued high-single-digit organic growth, UBS said in a note Tuesday. The firm pointed to coronary intravascular lithotripsy and renal denervation products expected in 2027 and 2028, representing a combined $2.5 billion served market, along with Empower leadless pacemaker launch in 2027 that could help the company gain share in a structurally higher-growth cardiac rhythm management market.UBS said management's lowered growth outlook for electrophysiology and Watchman is already reflected in Street models, giving the company room to shift focus toward other high-growth areas. The firm added that current share pricing implies sales declines through 2029, versus its own and consensus estimates for high-single-digit growth, suggesting the recent pullback is overdone.UBS assumed coverage of Boston Scientific with a buy rating and $74 price target.Price: $46.48, Change: $+0.97, Percent Change: +2.12%

Boston Scientific's 2026 Guidance Intact Despite Watchman Headwind, RBC Capital Markets Says

Boston Scientific (BSX) shares fell Wednesday after Chief Executive Mike Mahoney's comments about flat sequential US revenue for the Watchman device in Q2 and potentially in Q3, but the company's 2026 guidance remains intact, RBC Capital Markets said in a note. The investment firm said the stroke prevention device's performance reflects continued decline in standalone procedures, although this was partially offset by an increase in concomitant procedures.RBC highlighted that Boston Scientific's organic revenue growth guidance of 5% to 7% year on year in Q2, and 6.5% to 8% for 2026, including the flat outlook for Watchman, is still in place. Management also expects the Watchman device to "return to hypergrowth" backed by factors like a label change submitted to the US Food and Drug Administration, as well as patient education. The investment firm said it continues to believe in the long-term growth contribution from Watchman based on checks despite the near-term trends.RBC Capital Markets has an outperform rating on Boston Scientific and a $100 price target.Shares of Boston Scientific were down 2.9% in Thursday trading.Price: $49.02, Change: $-1.45, Percent Change: -2.86%
