Chair Ron Edmonds will serve as interim CFO while the company searches for a permanent successor and will step aside as chair during this period, per the filing.
Deputy Chair Robert Natter will assume the role of chair, the filing added.
The company's shares fell 2% in recent Tuesday trade.
