HCA Healthcare Delivers Consistent Message Around H2 Cadence Expectations, RBC Says

HCA Healthcare's (HCA) management delivered a consistent message around second-half cadence expectations, with a more difficult comparison ahead for Q3 and a relatively more favorable comparison for Q4, RBC Capital Markets said in a Monday note.

Amid short-term Health Insurance Exchange and Medicaid turbulence, the company's longer-term positioning for capturing market share remains encouraging, RBC said.

Management characterized the full-year adjusted EBITDA impact from health insurance exchange volume declines at $1.2 billion, driven by a nearly one-for-one migration of patients who were previously covered into the uninsured population, the note said.

Management expects Q4 adjusted EBITDA growth to exceed Q3, supported by a higher volume of Q3 state supplemental payment retro receipts, the impact of exchange volume declines in Q4, and the resiliency program delivering incrementally stronger cost savings in Q4 than Q3 as program maturity and execution timing improve, the note added.

Approvals for all five states that filed under the grandfathering provisions of the One Big Beautiful Bill have now been received, creating a higher starting point for Medicaid supplemental payment revenue ahead of the statutory reform period. This provides an incremental benefit to the current year earnings run rate and a more favorable baseline into 2027, the brokerage said.

RBC kept an outperform rating on HCA Healthcare with a price target of $435.
